You need a separate transformer for each output to break ground. But if they are for Hifi-gear they usually come in pairs.

The MPC1000 has unbalanced outputs and would need an external
convertor (like a DI box) to take advantage of a balanced cable.

so if it has unbalanced outputs and my soundcards inputs are balanced i will need a direct in box to fix my problem? sorry if i sound liek an idiot :(

I'm saying that something with a transformer in it is a way to break ground loops. A DI box can be one way to do this. But then you will need
balanced cables, too. An hifi audio isolation transformer usually has RCA
connectors which means cheaper cables.

If you don't have a ground loop but have hum anyway, like if you have put an external power transformer close to your cables, breaking the ground won't help ( but instead a balanced signal would).

Putting transformers in the signal path also slightly distorts the signal. Audiophiles probably would not do it. So other solutions might be preferable. Google for "ground loops".
