Anyone seen this before? It happened when i was experiencing several(possible) bugs during q event recording.

You discovered another bug in the MPC4000, assuming it hasn't been reported elsewhere already. It won't do much good to report this icon disappearance to AKAI unless you can reproduce the problem.
Taking a photo of it was an excellent idea. It shows others exactly the kind of minor bugs that are in the MPC4000. It's a great machine, but it has a few nuisances. This one looks like a simple OS problem.

I don't think i can reproduce it. But next time i will also make a picture and try if i can reproduce the bug.
