Here's a real head scratcher, so please bare with me.

I'm running guitar into my R input.
Level = 100, Pan R50, Out = ST, FX = FX1, Send = 100.
My stereo right output is being sent to a guitar amp.

I have some samples routed to my L output.
Levels vary, Pan L50, Out = ST, FX = FX2, Send = 100
My stereo left output is being sent to the PA.

My [Effects set 1] contains:
FX1: Delay, mode: mono, pan R50
FX2: Delay, mode: mono, pan L50

When I play the guitar, the signal is coming out of the guitar amp only.
When I play the samples, the signal is coming out of the PA *and* the guitar amp, for some reason.

Has anyone else ran into this issue?

I reported what I think is the same bug (enabling FX1->FX2 screws up stereo routing weirdly). From JJ:

The bug of the report checked.
However, this bug is unfixable.
Since it does not work normally even if it checks by OS of AKAI, a cause cannot be grasped.
We will continue the investigation, but there is no possibility of fixing.

apparently it's a bug in the original OS.
