Akai s950 with IB-109 scsi

The s950 OS. This SCSI card works only with the latest OS that came out. And this OS was provided only on Eprom not on floppy disks. But If you have access to a 950 with this Eprom and the technical equipment I guess you could read out the Eprom and burn it on another.
