Ok I think I found out what the problem was. Interestingly it was the same issue that happened on my Dreamcast many years ago.

The pad upgrade from MPCStuff has a big ribbon cable that sticks out to the side and pushes down on the Q-link ribbon cable. The pins in the socket that sends signals off the PCB (next to the bottom Q-link) need to be bent downwards slightly so that they continue to make a proper contact with the holes in the plug even when it is being pushed down.

Hope this saves some people from a headache.
