u just need to study producers. Get a feel for how they do shit. Sample a fewdiff loops from a song and use thouse as varitaions.

depends on what you want to achieve.

The loosest rule you should follow (or disregard if you want to) is to make changeups happen in multiples of 4 or 8 bars.

But, as these gentlemen said, its up to you.
