Ive got a 1/4 running from the headphone out to the RCAs on the back of my mixer, it was workin fine, but now I get a really low level when I plug it in, and it has a slight flange...When I take the 1/4 out of the headphones and let it sit half way it has full sound. Anybody know what the deal is with that?

that is because you're taking a stereo signal into a mono signal, you will get phasing problems.
